    Health Plan Changes

    Effective 1/1/2020

The Local 965 Health & Welfare Trustees remain committed to continued good health for all.  To that end, we continue to urge members and their spouses to have an annual physical and biometric screening.  Those items are at no cost to you and covered 100% by our plan.  As an incentive, if those things are done prior to October 31 each year and reported to HMC, you will receive a lower deductible during the following calendar year.   The following rates become effective January 1, 2020.

                                                    Deductible                            Out of Pocket Maximum

In Compliance with             $500 per person                    $1,500 per person

    Physical & Screening *

    Non-Compliant                    $1,800 per person                 $2,800 per person

*Both member AND spouse must be in compliance to obtain the Lower Deductible and Lower Out of Pocket Maximum.  If either the member or spouse do not, they will both receive the higher rates.

Effective January 1, 2020, your deductible will be included in the maximum Out of Pocket limit regardless of whether you are utilizing the lower or higher deductible.

All children will be granted the lower deductible rate.
